disable ident & dns lookups, using tcpserver's options to do that, and
then try again. check your logs

qmail-pw2u reads from stdin and writes to stdout
you have to use it like that
qmail-pw2u </etc/passwd >/var/qmail/users/assign

it sorts by message id's. This means, even if you change the subject,
the message will be recognized as in-thread
