Try running /var/qmail/bin/qmail-smtpd. You should see something like:

  $ /var/qmail/bin/qmail-smtpd
  220 sws5.ctd.ornl.gov ESMTP
  quit
  221 sws5.ctd.ornl.gov
  $

If that fails, show us the output of:

  ls -l /var/qmail/bin/qmail-smtpd
  file /var/qmail/bin/qmail-smtpd
  df /var/qmail/bin/qmail-smtpd
  ls -ld / /var /var/qmail /var/qmail/bin

Make sure the file system indicated in the "df" output isn't mounted
with the noexec option. But because you can run qmail-inject, I don't
think that's the problem.
