No.  qmail deals with mail messages, not files -- if you don't count
writing the messages to files in Maildirs or to mbox files.

Any "attachment" to a mail message is just part of the message body, as
far as qmail is concerned.  It's entirely dealt with by your MUA, not
qmail.

There are various scripts to filter out attachments of various
content-types.  See qmail.org for an example or two.
