Bruce Guenter's QMAILQUEUE patch can be used to arbitrarily filter
messages. You could even use a tcpserver rules file to set the
appropriate variable only for mail which is outgoing (well, originates
at a local IP address).
Bruce also has a package called qmail-qfilter which uses the above
technique to do various filtering of mail. You can likely use parts of
it for examples. See untroubled.org for his software, or qmail.org for
other links.
