Thanks, Tom, this was the problem and once I got rid of both ezmlm's and
installed only ezmlm-idx, I now have working lists! :)

You should only have ezmlm-idx installed.  Qmailadmin was configured
for ezmlm-idx, but it's calling ezmlm-make from the regular ezmlm
install (instead of the ezmlm-idx install).
