if you are using qmail with vpopmail+mysql then this is possible
vpopmail can store the last time the user checked his her email in to the
mysql database.
then there is a tool called vdeloldusers which can delete the user account
which havent been used for X days. I guess it wouldnt be a big deal which
shows the last usage time of an account via an simple cgi script which can
access to mysql database if you need to.
or you can put the vdeloldusers script to cron to delete users which didnt
use their emails for x days
