All systems store info in a mysql database and work just fine. When I try
and update a quota size for a user on a virtual domain using qmailadmin it
populates the mysql table just fine, but it appears to not update the
maildirsize file in the user's Maildir.

I have tried to check mail via pop and imap and also sent mail to the
account, but it does not update.

Consequently, when using imp or imap to check the quota usage imapd thinks
that the quota usage has not changed. When I remove the file, it checks the
db just fine and updates the file.

I am assuming the responsibility is on qmailadmin to update the quota there
also. So, the question is - short of manually removing the file or
scheduling a cron job to wipe out the folders periodically, is there a way
to have qmailadmin update the file?
This is not yet a supported patch. Qmailadmin is probably not deleting
the file, then recreating it with the new quota.
