The QmailToaster doesn't allow for user/pass in the smtproutes. There
is a patch but it is for a very old version of Qmail and is probably
not going to be added to the QmailToaster proper.

My suggestion to you is to put ":mail.pacbell.net" into
/var/qmail/control/smtproutes.

This mail server accepts all sbc addresses and doesn't require smtp auth.
