This is way too high. In a spam attack your server will be in tears.

You didn't set a postmaster address in /var/qmail/aliases. There sound
be a few .qmail- files that need editing.

Outlook configuration problem, most likely. Might want to test with
Thunderbird or the webmail to see if Outlook is the problem.

I'd recommend updating squirrelmail too. The clamav definitely a good
