It may not be these perms as I have the same on my QT and I'm working OK.  I also use SMTP auth to send via my QT and it works great.  In fact the only thing I can't get working (and not sure it's in QT) is the pop before smtp patch so you don't actually need to SMTP auth - you can simply pop mail first and have the server remember you IP for a period of time which gets added to the list of relay clients.

So this error is when you are using SMTP auth for sending from remote locations?

What happens when you telnet to your machine from a remote IP (i.e. one that's not in your tcp.smtp) and try to send?
