I've no experience with the older versions, so I'm of no help here.

You definitely need to configure domainkeys. See
/var/qmail/doc/README.domainkeys or
http://www.qmailtoaster.com/qt-doc/QT-README.domainkeys
If you want to run w/out domain keys, simply remove DK* variables from
/etc/tcprules.d/tcp.smtp and rebuild the tcp.smtp.cdb
Spamassassin, ClamAV and Simscan are totally new friends and I have
not configured them in any way (should I?).
They work pretty well with the default configuration.
Is there Qmailtoaster specific documentation / tutorials available for those?
I don't know of any that's toaster specific. Someone *please* correct me
if I'm wrong (I'd love to see better documentation!). Google is your friend.

.) I don't know the status of the *-debuginfo-* packages, or whether or
not they work with the newer base packages. My guess would be that they
don't. They're not necessary. I'd remove them.
Will someone (J/E/N) elaborate on the status of these packages? I can
have the new upgrade script remove them if that's appropriate.
.) horde-toaster is not part of the current toaster. I see from the list
archives that squirrelmail-toaster replaced that functionality in
version 1.1. I'll modify the new upgrade script to remove horde-toaster.
I also see that horde (not horde-toaster) appears to work ok with the
toaster, if you require it for some reason. See the archive list for
details.
.) I'm not sure why you have qmail-toaster-1.03-1.0.15 still there. I'm
guessing it's because of hoarde-toaster.
