Well, the install was done with the qmailtoaster script files for fc5. Configuration appears to be the same as our other servers. the locals file only has localhost and the actual hostname in it. For the life of me I couldn't find any reason for the need for .qmail for each user. The defaultdelivery file is correct as far as I can see. "./Maildir/" The .qmail files are "./username/Maildir/", the only thing I could think of was that the default delivery wasn't interpreting the usernames correctly, but no idea why that would be.
So it is wrong that I need to create the .qmail file for each user? It's not maybe a problem with simscan? To get around the annoyance of having to manually create the files, one of our software guys created a wrapper for me, that creates the file on user creation automatically, but it would be nice to have a system that works as it should.
