The "scanners:none" seems to be where your problem lies.
It should say "scanners: attach: 1.2.0 clamav: 0.88.3/m:39/d:1628 spam: 3.1.3" or some such.

So I think it's safe to say that simscan isn't invoking the other scanners.

That sure looks ok to me.

You said you regenerated the simcontrol.cdb file. What command did you use?
# qmailctl cdb
Reloaded /etc/tcprules.d/tcp.smtp
Reloaded /var/qmail/control/badmimetypes.cdb
Reloaded /var/qmail/control/badloadertypes.cdb
Reloaded /var/qmail/control/simversions.cdb
Reloaded /var/qmail/control/simcontrol.cdb

Did you get the last simcontrol message?
