Chkuser doesn't seem to be getting the rcpt address. I was thinking
permission errors but /var/qmail/control is uid/gid of root/qmail and
tcp.smtp/tcp.smtp.cdb owned by root and is world readable. tcp.smtp.cdb

You went too far separating lines. :(
You should have 3 lines. The first one is ok. The second and third should be
joined with a comma separating. The 4th through last lines should be joined
with comma separating.
