I found this identical problem (Yahoo!: Please Verify Your Email).
I've narrowed it down to DK processing. (Josh, did you remember to 'qmailctl cdb' after changing tcp.smtp?)

Here's what I observed:
The message from yahoo would appear from the logs to be coming through ok, but the message never arrived. Some period of time later (10-45 minutes) yahoo's server would try sending again, which leads me to believe that the smtp session wasn't ending normally. I added an tcp.smtp entry for the particular yahoo server it was coming from, removing DK variables, and on the next attempt it came through.

Conclusion: DK processing was failing/rejecting the message. So I look at message headers, expecting to see an invalid DK signature. Here is the message header:

Guess what? Apparently no DK signature. Anyone have any idea(s) as to why DK checking on incoming mail would reject or fail on this message?

Hi Guys

Im still having some issues with this.

I set myself up a yahoo email address and tried to get my password
conformation etc sent to my account at my toaster and another account.

The email was received at the other email account just fine, however not
for the account on the toaster.

After some investigation, I found the same issue that I originally
reported. It shows up in the SMTP log, but doesn't get delivered to the
mailbox.
