I'd look into /var/qmail/queue. I think it's the 'mess' subdirectory,
but I'm not sure about that. Could be 'info'.
I would also check disk space to make sure you're not out (had that
happen to me once), and do a hard reboot (fixed mine another time that
had happened).
Didn't fix, still fighting same problem.
