I hate responding to my posts, but here is what I think I figured out-

I use RulesDuJour script and I started adding some more in for use. I did not realize how huge the blacklist script was. It caused my spamd processes to heavily swap the server (it is slow and old and will be replaced). This caused all sorts of spurious errors. I ditched blacklist and blacklist_url and things are much happier now. Only run those lists if you have lots of RAM or very few spamd children (I run about 30 to keep up with my load on that box - they cannot have too big a footprint).

I have a qmailtoaster and it has been running well for a while. I have had to restart the box and now I see some strange behavior:

1. Times to respond for smtpd are slow - it can take quite a while to get the greeting - I have a concurrencyincoming set to 100 and do not see that many qmail-smtpd processes

2. Spamd runs, but my mails are not being marked with [SPAM] - in fact none of the previous headers are being set in messages. Logs look ok for spamd/clamd - is there something else in simscan to look at? I am getting local delivery, just no spam

Any help with #2 especially is welcome.
