Are the messages on this server being scanned? (Look at received headers for X-spam stuff).
What does your /etc/tcprules/tcp.smtp contain? It should have QMAILQUEUE="/var/qmail/bin/simscan" defined so that messages get passed through simscan. I've seen this problem before, but don't recall a specific fix. Could be a number of things.
