Add some additional rules from rulesemporium.com, enable URIBL from the wiki, train your Bayes (some info also on the wiki), create some custom rules, and install some additional blacklists.

I'm not sure what you already have in place, so I tried to hit all the bases. You'll find that blacklists and URIBL will be the biggest bang for your time. Unfortunately blocking spam is not a cut-and-dry thing. What works for me may not work for you, and neither of our solutions may work for someone else. There's a lot of "playing and tweaking" that you'll end up doing to get it the way you want.
