If I remember correctly, incoming messages will (if you turned it on) be run through the mailfilter script after being scanned by clamav and spamassassin. They then get delivered to your mailbox, where if you have a forward, then sends a copy to a new address using the INJECT function, so it's not scanned a second time by the second receiving account. I haven't been keeping an eye on the thread, so can you detail what the issue was again?
