I decide to try after reading it: 
http://wiki.qmailtoaster.com/index.php/Smtproutes
It woks fine with my test qt from latest stable rpms.
On my test machine in /var/qmail/control/smtproutes was added only one line:

:smtp.myisp.com user userspass

(of course we must have some account on isp's smtp server :) )

qmailctl stop
qmailctl start

Aftet it when some user use my test qt as relay, test qt use my isp's smtp 
server for relaying.

Looks like all works good. :)
