in your /var/qmail/supervise/smtp/run
what you got for :
TCP_CDB="/etc/tcprules.d/tcp.smtp.cdb" ??
does that match what you got ?
and check your tcp.smtp.cdb file
do something like "strings /etc/tcprules.d/tcp.smtp.cdb" w/o the "" of course
and check if that matches the 2 lines Warren told you to use

It's look like that Qmail is ignoring rcpthosts file, it is relay to any domain from any domain even after I delete all lines from /etc/tcp.smtp and rebuild the cdb file.
