Mark, There have been several discussions of this problem on the list in the last couple of years. I suggest reading http://qmail.jms1.net/test-auth.shtml to see if your test routine is REALLY testing SMTP-AUTH or not.
If following the steps listed there doesn't clarify the problem, check the list archives the 'SMTP-AUTH'.
