There's really no 100% reliable way to keep off of blacklists, but I
would recommend setting up an outbound spam filtering gateway.

I set up a MailScanner (http://www.mailscanner.info/) Virtual Machine
expressly for this purpose. It was pretty easy to set up, is
well-documented, and has a very (VERY) active community mailing list.

I then installed MailWatch (http://mailwatch.sourceforge.net/doku.php)
as a user interface for that service, as it allows you to see the
disposition of any given email that has passed through the system.
MailWatch was a chore to install, but once it's working properly, it's
extremely useful.

All of my web-servers, mail servers, etc, relay mail through my
anti-spam and anti-virus gateway before hitting the internet, which has
noticeably reduced the occurrences of my mail servers appearing on
RBL's.
