do those computers 1) send via your server (which is located in one of those
problematic domains) or shall your server 2) receive mails from those machines?
In case 1) I recommend to make qmail all mails forwarding to the provider's
regular SMTP server.
In case 2) your users should better use the submission port 587 with
authentication to send mails.
The problem is in case 1) that even when you whitelist those IP-Addresses,
other many other recipients will not be able to receive those mail, because
many providers are using the spamhouse blacklists...

So, the question is how can I set up exceptions, or just disable the
outgoing check completely - either for specific accounts, or just system
wide?
Clearing out, for example, spamhaus from /var/qmail/control/blacklists
made no difference to this.
