I have done that for a few companies
a couple of QMT machines acting as smtp filters , antispam/antivirus/rbl ...
frontend to a few other QMT machines acting as pop3/imap4 machines and outgoing smtp for the customers,
and some exchange servers

You just need to know how smtproutes work
and add the domain to either rcpthosts or morercpthosts (I use that one) and a quick 'qmail-newmrh' and you are done
this is of course done on the frontend filtering machines.

You can of course use load balancing policies or nas/san to hold your mailboxes ... you just route the "clean" mail where you want

People have done this as a spam filter for Exchange servers, but I have not seen anyone do it for a QMT machine as the backend. If you search the archives you'll find a few ways to accomplish this.
