On the same RHEL 4 box where I've been seeing clamd swell up and push the load on the box to 10, I'm seeing instances where "qmailctl stop" is leaving qmail-send, qmail-smtpd and other qmail service running after the stop command is issued.

it may happen when you type qmailctl stop the server is receiving connection or try to sending email to destination if you see qmailctl stat it will say something like qmail-send: up xxx seconds (going to down)
it will eventually die after the server succesfully send to destination.
so I am never type qmailctl start right after qmailctl stop..
I'll make sure all services stops before run it again
otherwise qmail-send will complaining "it can't run because it still running"
then I can get myself in trouble...
