I have seen this behavior when chkuser fails... personally, since I delete all mis-addressed emails anyway, I turn off chkuser...

To try this, edit the /etc/tcprules.d/tcp.smtp file, and add the following to each line you want affected:
SENDER_NOCHECK="1"

Personally, my LAST line looks like:
:allow,BADMIMETYPE="",BADLOADERTYPE="M",CHKUSER_RCPTLIMIT="50",CHKUSER_WRONGRCPTLIMIT="10",DKVERIFY="DEGIJKfh",QMAILQUEUE="/var/qmail/bin/simscan",DKQUEUE="/var/qmail/bin/qmail-queue.orig",DKSIGN="/var/qmail/control/domainkeys/%/private",SENDER_NOCHECK="1"

I have NO IDEA if this makes for a "best practices" or not -- I'm just one guy who has admittedly patched up my QMT for my own devices!

Just a reminder, you have to do a "qmailctl cdb" after mucking around with the tcpruled.d directory in order to make your changes take effect.
