Duplicates typically result when an smtp session terminates before the
sending server receives a successful acknowledgement (the session ends
normally). The sending server then (appropriately) resends the message after
some period of time.

Timeouts of any kind can cause this behavior. If your toaster is taking a
long time to scan (3-5 minutes), this would likely be the culprit, as the
smtp session remains active during this period, and the sending server may
time out since there's no smtp traffic going on during the scan.

I would highly recommend upgrading your toaster packages as a first step.
You should probably update your OS packages (if not the OS release) as well,
but do that before upgrading the toaster.
