Hm. Possibly a bit off topic for the list, but the short answer is "just like forward DNS, but instead of <name> IN A <address> the record looks like <address> IN PTR <name>."
Check out sections 5.3 and 5.5 of the DNS howto at: http://tldp.org/HOWTO/DNS-HOWTO-5.html That should give you a good overview of the process and potential gotchas. You may also ask your network provider whether they control the reverse zone files or if they are delegated to you.
