The way I read the docs on this is that the last line is a config for
everything else. I just was unclear of direction.. Does this work for
both ways when configuring for a domain?

You are correct - the : line matches everything not already matched
previously.
Your config will allow their domain to *not* be scanned for
viruses/spam, same for wletc, with the addition of also blocking some
attachments. Everything else gets spam scanned, but not virus scanned
(you have clam=no for all domains, including the catchall), with
messages scoring over 10 being deleted and also blocking some
attachments.
