#2, I have messages in there that date back to Jun'08. Is there no
logrotate configuration for this log as part of the package?

#1 - I'll assume you have quotas off for that account, correct?
Yes. I'm seeing it on 2 accounts, both of which are very large (multiple
gigs of mail).
#2 - No, it's not in logrotate. Erik threw it in a long time ago and I
have never gone back and looked at it.
If you think it should have a logrotate entry, open a ticket for it.
I think every log should have a logrotate entry, but let's not waste any
time on either of these. I'd rather just make a note of it, and work on
getting 'deliver' (dovecot's maildrop replacement) working with QMT. Ok?
