I have double checked and the user AND domain are both in the QControl admin utility and the domain is in the /var/qmail/control/virtualdomains file.

Where else should I look to debug this? This happened to email address in the domain (same inbound email).

Is the domain in /var/qmail/control/rcpthosts ?

If so, is the domain in /var/qmail/control/users/assign ?

Yes, the domain is in both files on both servers.

Is this a 1-time deal, or is there any consistency?
Is the server receiving other messages for the domain? user?

Since this has not happened again that I am aware of in over 24 hours, I'm gonna call it a one-time wonder and be done with it. I'll worry more about it if it happens again.
