Are you using spamdyke (recommended), or rblsmtpd?

Not that it's related to your problem, but I expect that there's very little benefit to a couple of these. Along with spamdyke, I use only zen.spamhaus.org and bl.spamcop.net.

Also, you should be running a caching nameserver on your QMT host.

I'd double check the rules on these devices. I'm betting that's where the problem is at this point, and there's some sort of rule somewhere that you've missed. The symptoms that you're seeing fit this as being the problem.

btw, all mail server are in same subnet

    Off hand, this 'feels' like a problem with a router/firewall device.
    Are
