Many "residential" ISP services (particularly those from AT&T and Verizon) block outbound activity on port 25 in an attempt to combat SPAM (and to block "infected" systems from sending massive amounts of e-mail that way).

That's why I setup my QMTs to listen on ports 587 (auth, but no SSL required) and 465 (auth and SSL required) - SOME of my clients insist on SSL connections for outside access, thus the reason for the 2 ports

The WIKI has steps to show you how to make sure you're listening (qmail-smtp) on the extra ports, as well as how to force AUTH and/or TLS/SSL on the connections. Personally, to avoid the "it works on my laptop at Starbucks, but not at home" issues, I always configure IMAP connections to use ports 993 & 465.
