Looks as though clamd's running ok. Next thing to check would be simscan.

Let's have a look at:
# cat /var/qmail/control/simcontrol
# cat /etc/tcprules.d/tcp.smtp

Have you run "service qmail cdb" recently?
This needs to be run whenever the above 2 (and a few others) are changed.

    The qtp-newmodel script is historically quite reliable. There's
    always a possibility of a situation though that could reveal a
    problem. There's a lot of code involved with that (>3000 lines if my
    memory serves).
