I don't necessarily agree that a Smart Host is designed for filtering.

I've always thought that a smart host was pretty much synonymous with a
MSA, but I don't see why it couldn't be extended a bit to incorporate
some filtering, as sort of a reverse proxy for mail. In a stock QMT
configuration though, authenticated emails are only scanned for viruses.

There appears to be two parts to this request.
1) being able to filter outbound spam as well as incoming
2) being able to do spam filtering selectively by domain
Regarding #1, simcontrol handles what gets scanned. I don't recall off
hand the mechanism used here. I do anticipate though that QMT might be
changing to amavisd-new at some point in the fairly distant future. You
might want to look into what amavisd-new can do in this area.
#2 can be done presently with simcontrol, but I'd look to amavisd-new

SPAM FILTERING is a fairly broad term. If you're looking to implement
distinct filtering rules per domain (above and beyone on/off), that's
another matter as well. This would be a very nice feature which I expect
MSPs would love to have. I'll be keeping this in mind as I attempt to
loosen the coupling of the QMT modules.
As far as outbound spam is concerned, I'm very interested in developing
a throttle on qmail-remote. This has been mentioned fairly recently on
this list. I think such a feature will work wonders controlling outbound
spam. Much more so than the overhead and complexities involved with
attempting to scan outbound messages.
