In the error_log I found only this:
PHP Notice:  Undefined index: file in /usr/share/toaster/include/admin.inc.php on line 5

But I've seen this error also on functioning VQADMIN.

I haven't installed Qmailtoaster on CentOS 6 myself, but if elements are missing from a web page, I would perhaps start looking from Apache logs for more information?
