It's in the myuser directory, next door to the Maildir.

.qmail-default only comes into play when the user isn't found (catchall). (TTBOMK)

Yes (and sometimes erroneously, like when it changes the name in the account it creates a duplicate delivery record).

Yes, QMT will be using dovecot-lda as well. I'm not sure at this juncture how that's going to be implemented. I imagine that it'll be via .qmail files, but I can't say for sure.
