Just an update. I've looked into this, and the PVE host appears to be
having some disk issue that is severely affecting performance, which is
resulting in smtp session timeouts, which subsequently results in dupes.

I believe this problem has been fixed. The (hardware) raid array on the
host (the list server is a VM) had some problems, and the VM has been
migrated to a healthier host.
Just an FYI, there was another symptom in addition to occasional
duplicates. qmailmrtg, which runs every 5 minutes, was also occasionally
sending out warning messages (email notifications from cron) that
qmailmrtg was already running. FWIW.
