Helmut's spot on. In order to tell why something's marked spam, don't
guess. The headers (and spamd log) tell you exactly which rules fired,
and what value each rule scored. Only after you know this can you take
predictable action. You may simply need to adjust your required_score
parameter a little upwards.
I have googled and -- not found a good doc on how to check the spamd log

How might i find that?

I suspect Google purged all COS4 related content since it's EOL. (JK)
The easy way to access (legacy) QMT logs is with the qmlog command, such as:
# qmlog spamd
Simple as that. You can also use -f flag to follow the log (like the
tail command), and there are various searching flags as well (-lc is
very useful).
If you don't have QTP installed yet, you can get the rpm for it here:
http://qtp.qmailtoaster.com/repos/nodist/
If you'd rather do things the hard way, you can poke around QMT log
files in the /var/log/qmail/ directory.
Thanks.
