Personally, I do all of my INTERNAL relaying over secure logins...

FIRST, on the server that will be /sending the /internal message, I create an SMTPROUTES rule that sends it to my REAL mail server -- but here's the kicker: the SMTPROUTES rule says to use port 587 and to use a set of CREDENTIALS that are setup for internal mail transfers.

In my case, on the "real" mail server there is a "fake" domain called INTERNAL.MAIL, with a "fake" user of INTER-SERVER... Thus, my internal mail transfers use port 587 with auth credentials of [email protected]...
In other words...

   /var/qmail/control/smtproutes:
   *mydomain.com:realmailserver.mydomain.com:587
   [email protected] PASSWORD*
