Jeff,

This can happen for several reason.

1) If you are on a mailing list and someone sends an email to you and the 
mailing list, there will be 2 emails.
2) If the message is queue on the server and the connection is interrupted 
before the client gets a reply to the quit command from the server the remote 
server will resend the message.
3) There are two instances of Dovecot running on different hosts.

You might increase the timeout in spamdyke.

This is all I can think of now.
