That was it. I needed to add the image object to the application at startup. I was always removing and adding the QxImage. That is much happier now.

Sounds like your image is not added to the document or any visible widget at application startup? qooxdoo is smart in preloading all _visible_ images first, then finalizing the main application, then preloading all _invisible_ images. Last step is triggered by user interaction, e.g. moving the mouse. Make sure you have made the image visible initially. Maybe you could also post a short testcase?
