The creation of the Modern theme also started with Photoshop images of all qooxdoo widgets. So this would be a good starting point. How hard it is to implement a new theme from scratch depends heavily on how your theme looks like. With decorators you can style the widgets when you need to draw a border, showing a gradient or a shadow and stuff like that. For simple widgets you can also use backgroundColors and needn't to use a decorator at all. However, this depends on how your theme looks like. For example you can compare the two appearance theme files of the Classic and Modern theme.
