Actually i'm using the same kind of header in qooxdoo app.
What i did is as simple as this
var layout = new qx.ui.layout.HBox();
layout.setAlignY("middle");
var header = new qx.ui.container.Composite(layout);
header.setAppearance("app-header");
Then i add this header to a main container on the north edge.
For the footer it's exactly the same thing but i add it to the main
container south.
