Change use of AMQConnectionException

Change AMQConnectionException to be analagous to AMQChannelException, (i.e.
representing an exception on a connection, rather than a failure to connect).
Use the new AMQConnectionFailureException to represent the previous meaning of
AMQConnectionException.
