You're right.

I think we should emulate XMPP (Jabber's) approach
http://www.xmpp.org/extensions/xep-0178.html

It also means we're not fully formed in the AMQP specification wrt
specifying how TLS gets started.

Through EXTERNAL, yes, but maybe I'm missing something but I don't see
anything put up that would require a client to manage the SASL
authentication exchange using EXTERNAL instead of some other SASL
mechanism.
