StateManager fails to change states from Connection_Tune to Connected

Related to QPID-330 in that this was the reason it was "missing" the connection.
Closed QPID-330 as it was related to AMQConnection which has been tidied up so
that it won't hang the connection.
The StateManager will now report its timeout in attaining the desired state.
