Modified C++ examples - comments, exchange configurability, XML Exchange naming

I modified the C++ examples:
1. Comments in each file are now accurate.
2. You can no longer choose an exchange via the command line. The direct
examples require the "amq.direct" exchange, etc.
3. Naming changed for routing key in C++ XML Exchange example - it's now
"content_feed". Don't want to imply that you can only have a single binding for
a given binding key.
